Chronic Care Management (CCM) is defined as the non-face-to-face services provided to Medicare beneficiaries who have multiple (two or more), significant chronic conditions.
Chronic care management encompasses the oversight and education activities conducted by health care professionals to help patients with chronic diseases and health conditions such as diabetes, high blood pressure, lupus, multiple sclerosis and sleep apnea learn to understand their condition and live successfully with it.
One of our main goals is to keep you the most healthy we possibly can by answering questions each month and working to avoid ER visits and hospital stays.
This term, chronic care management services, is equivalent to disease management for chronic conditions. This work involves motivating patients to persist in necessary therapies and interventions and helping them to achieve an ongoing, reasonable quality of life.
An estimated 117 million adults have one or more chronic health conditions, and one in four adults have two or more chronic health conditions. Chronic disease is prevalent among Medicare beneficiaries, with most beneficiaries having multiple chronic conditions, which increases the risk for poor health outcomes such as mortality and functional limitations.
Some of the functional limitations that can occur are caused from becoming more and more sedentary, having diabetic ulcers, not being able to do one’s daily activities such as cooking, cleaning, bathing easily and other household activities of daily living.
Another goal that we have is to help each person participating in this service to remain active, learn to prepare healthy meals, follow diet and exercise plans, not miss important appointments such as eye appointments or physical therapy appointments and the like.
There is a fee every month of up to $8.00. Some people may qualify for much lower fees. We would be able to let you know what your fee would be.
